Care Plans/GP Management Plan
We provide comprehensive and holistic care plans to patients with chronic medical conditions. The criterion for eligibility is governed by the Medicare. If you have chronic ongoing medical conditions, your GP may suggest a GPMP. If you also have complex care needs and require treatment from two or more other health care providers, your GP may suggest TCAs as well. General Practitioner Management Plan (GPMP) identifies the patient’s health and care needs, sets out the services to be provided by the GP, and lists the actions the patient can take to help manage their conditions.
Each year you are entitled to a GP Management Plan(721) and 3-4 reviews (732),as determined by your GP. Most patients with GP Management Plans are entitled to 5 subsidised allied health visits per calendar year under the Enhanced Primary Care (EPC).This can be used for visits to a podiatrist, physiotherapist, exercise physiologist, speech therapist, occupational therapist, osteopath, dietitian, audiologist, diabetes educator, mental health worker or aboriginal health worker.
